What to Bring to Your Interview
You can help us get you into our system as quickly as possible by coming to your interview prepared.
- Dentists should bring:
- Your current CV.
- Your social security card. If none is available, then bring a passport or birth certificate. (If you do not bring your SS card, then we will need to know exactly how your name reads on it.)
- Your Alien Registration Receipt Card (green card) if you are a lawful permanent resident.
- Your current driver's license.
- Your Hepatitis B immunization records. The least we need is the complete dates of your three shots, however we would prefer you bring actual documentation of immunization.
- Two or three professional references (former employers, instructors, etc. No friends or relatives, please). We would prefer to talk to these references, so please bring their phone numbers, if possible.
- Your current North Carolina dental license renewal.
- Your DEA number.
- Your malpractice insurance.
- Your privilege license.
- Your NPI number.
- Your Medicaid number, if you have one.
- Hygienists should bring:
- Your current resume.
- Your social security card. If none is available, then bring a passport or birth certificate. (If you do not bring your SS card, then we will need to know exactly how your name reads on it.)

- Your diploma from an ADA accredited school, if you have graduated from one.
- If neither of the above applies, we will need
- Record of your x-ray certification that meets the North Carolina requirements and
- Record of your completion of a three hour course in sterilization and infection control (OSHA) and
- Record of your completion of a three hour course in dental office emergencies.
- Your coronal polishing certificate.
- Your CPR card.

- Your current resume.
